Below are some forms that are available for you to download and print. If you are a new patient, you can complete the Patient Registration and Photography Release form in advance to save you time at check-in. Otherwise, please arrive early so you can complete the necessary paper work.

Our office is open from 8:00 AM to 5:00 PM Monday to Thursday, and Friday from 8:00 AM to 12 PM. Please call our office during normal business hours at 704-663-1800 to speak with our office staff who will ask for the information necessary to set up your appointment.

If you are an existing patient, and you experience a dental emergency after our normal business hours, please call our main number and our recording will advise you how to proceed.

Financing & Payment Options

As a courtesy to our patients, we file for all insurance plans. To find out if we accept your insurance plan, give us a call, or arrive for your appointment early so you can discuss this with our billing department. For those who are interested, we offer financing through CareCredit and LendingClub. We also offer an in-house membership plan.
